Engaged in Melee

What does "engaged in melee combat" actually means?
If a creature just moves in melee range with an opponent, are they considered "engaged"? Or the creature that moves in melee range has to attack as well and THEN they are considered "engaged"?

It's a judgement call, but the way I handle it is if a creature moves close enough to melee attack, with the intent to attack, then it's "engaged." Initiative and actions are somewhat abstract and things are really sort of happening all at once.
