Ebonyr Campaign Setting: A fantasy setting by Knightvision games. The world is a combination of unique steampunk and magic that is just exiting a very dark time. Realms of Crawling Chaos: A Lovecraftian take on Labyrinth Lord. In R'lyeh, Cthulhu Stirs... Evil cultists consummate their union with Shub-Niggurath in dark woods no other mortals dare go. Alien terrors lurk in the underworld; their vast riches wait, or their incomprehensible powers could leave you broken, dead, or worse. Adventure in underworlds filled with horrible entities, or the ruins of alien beings that enslaved the ape-like ancestors of mankind. Seek hidden secrets from the arctic wastes to dark, briny seas. But as you approach the ocean, what are those strange longings you feel? Realms of Crawling Chaos is a Lovecraftian Dark Fantasy campaign supplement for Labyrinth Lord and the Advanced Edition Companion.
Red Tide: Campaign Sourcebook and Sandbox Toolkit: An alternate campaign setting by Sine Nomine Publishing. A Red Tide is Rising. For three hundred years, the last remnants of humanity have clung to the wild green jungles of the Sunset Isles. They have watched the red mist that consumed their world roil a hundred miles off the coast, waiting, a beast at bay. But now the crimson dreams are becoming stronger, and the wild Shou tribes of the west howl their hate at the human invaders. Who shall rise to save a world quenched in blood?
Shatterworld: A dark fantasy campaign setting under construction by Niccodaemus. Shatterworld is intended to be a low magic setting, where most people have never even seen an orc, a giant, a dragon, or a magic spell being cast. Influences include Middle-earth, Shannara, Thomas Covenant, Earthsea, Narnia, and Xanth as well as traditional folk tales along the likes of the Grimm brothers, The Ulster Cycle, Andrew Lang, and various songs and poetry. In these stories, there is an eerie otherworldliness to encounters with creatures such as elves, dwarves, etc., and it is this feeling Shatterworld attempts to evoke.
The Chronicles of Amherth: An alternate campaign setting heavily inspired by the Mystara and Ravenloft campaign settings. The Chronicles of Amherth is a Labyrinth LordTM campaign setting designed to provide new Labyrinth Lords with a ready-made, and most of all, manageable fantasy setting from which to quickly launch a new campaign. World history, backstory, timeline, and metaplot have all been kept to a minimum in order to help facilitate this goal. Amherth accommodates most of the well-known fantasy tropes including wizards, elves, dragons, dwarves, and many others. Veteran fantasy gamers will find much of the material contained within is easily adapted for use with existing campaigns.
The Lands of Ara: A richly detailed Labyrinth Lord campaign setting from carterscartopia.

I started one a while back but didn't complete the modules section. Updating it is on the never-ending todo list.

A page specifically targeted at settings and adventures would be great.

Great list, Mark! Good work on that. I can imagine that completing/maintaining the modules section would be a job.

To be fair, it wasn't nearly as complete when I replied earlier. I did a significant update to the page. Everyone is welcome to copy and paste the data into a more useful format as starting place. I'll be happy to include publishers/modules/etc. that I have overlooked as well.

You missed one of the most popular LL dungeon modules:

Stonehell Dungeon: Down Night-Haunted Halls
by Michael Curtis, author of The Dungeon Alphabet, co-author of Realms of Crawling Chaos
Stonehell Dungeon is a classic-style megadungeon, filled with enough monsters, traps, weirdness, and treasure to keep you gaming for a long, long time. Explore over 700 rooms, encounter more than 40 new monsters, and discover 18 mysterious magical items -- and that's just in the first book! Stonehell Dungeon: Down Night-Haunted Halls details the first six levels of a megadungeon intended for use with the Labyrinth Lord™ role-playing game, but is easily adaptable to most early versions of the original fantasy role-playing game and its retro-clones. Featuring art by J.A. D'Andrea, Lee Barber, Marcelo Paschoalin, and Ralph Pasucci, Stonehell Dungeon gives the game master all the necessary information to run his players through the dungeon, while offering enormous opportunities to customize and expand on the site. Suitable for characters of levels 1 through 7.
PDF HERE
There's a Free 6-Page Preview available.

There's also the free Stonehell Dungeon Supplement One: Brigand's Caves.
The first in a series of supplemental material for STONEHELL DUNGEON. This six-page PDF details the cave hideout of a group of brigands who lurk near the dungeon's entrance. It also introduces two new monsters for your Labyrinth Lord™ game and provides the PCs with the first hint of Stonehell Dungeon's dark secret.

And you can buy Stonehell Dungeon Supplement Two: Buried Secrets
The second in a series of supplemental material for STONEHELL DUNGEON, this twenty-page PDF is an anthology of three adventuring sites located in and around the ancient prison suitable for exploration by low-level characters. Included are “Modnar’s Cellar,” the undercroft of a ruined tower owned by a magic-user with a strange agenda, “The Nest of Otrogg,” a cave system once occupied by a loathsome cult, and “The Sanctuary of Chthonia,” a secret sub-level located inside Stonehell Dungeon and overseen by an undead witch. It also introduces two new spells, eight new monsters, two new magical items, and a pair of minor deities for your Labyrinth Lord™ game. This supplement contains three maps but no other interior illustrations.

There's the excellent Red Tide setting, by Kevin Crawford, author of Stars Without Number, available in PDF or POD.
A Red Tide is Rising
For three hundred years, the last remnants of humanity have clung to the wild green jungles of the Sunset Isles. They have watched the red mist that consumed their world roil a hundred miles off the coast, waiting, a beast at bay. But now the crimson dreams are becoming stronger, and the wild Shou tribes of the west howl their hate at the human invaders. Who shall rise to save a world quenched in blood?
In this book a Labyrinth Lord will find all that is required to run a sandbox game of adventure in the Sunset Isles. In addition to the races, classes, and sorcery of this savage land, a referee will find special tools and resources for creating shadowed courts of quarreling nobility, cities rife with struggle, wild border settlements that cry out for the help of heroes, and dark places in the earth known only to the dead and the damned. These tools will aid not only campaigns in the Sunset Isles, but also those games set in other lands of savage mystery and bloodstained blades.
Expanding on the tag system found in the free Stars Without Number sci-fi role-playing game, Red Tide includes more than fifty new tags for fantasy cities and borderland settlements, along with more than 25 pages of GM resources including culture-specific name and place lists, unkeyed blank maps for quick sandbox use, four pages of room dressing and treasure possibilities, and a new "diagram dungeon" method of quick dungeon generation with a worked example.

DwD Studios the folks behind Star Frontiers Digitally Remastered, have produced some nice modules recently:

Dark Times in Brighton
Dark Times in Brighton is an adventure for four to six characters of 1st - 2nd level. Although designed as a Labyrinth LordTM compatible product, It is easily compatible with most fantasy RPG systems. In addition to the adventure itself, contained within these pages are details of Brighton and its people, a town that could easily become an important part of your character's careers as adventurers.
The town of Brighton exists in the north, at the source of the Daakencraags where the Darkenwolde Forest shades the winding Brother’s River. The legendary adventurer, Richan Thatcher (along with his party of adventurers) carved the place away from a once mighty goblin kingdom. It has prospered for many long decades, until now.
Townsfolk are disappearing from the surrounding countryside. Goblins are on the march once more. If that wasn’t enough, a terrible blighting disease has infected the waters and not even the curative magics of the Temple of the Winds can thwart it. This is indeed a very dark time for Brighton.
Too old to do anything about it himself, Thatcher has sent out a call to any adventurers who will swear their swords to his mighty town and its innocent people.
Are your players hero enough to face down a growing goblin horde? Are they brave enough to fell the mighty goblin shaman, Vir-Kayik? Are they clever enough to find the source of the blight and cleanse the town’s water source? Are they the light to end these dark times in Brighton?

It's available in PDF or POD (the latter via Lulu) and is very nicely produced.

They've also done a couple of 1-sheet modules, available as pdfs:
Maidens of Moordoth
Maidens of Moordoth is a low-level 1-sheet adventure designed for Labyrinth LordTM but easily usable in any old school fantasy role-playing game. It provides all you'll need for a full night or two of play.
The elders of the village of Moordoth are the keepers of a grim secret. They vowed to take it to their graves, but a pack of ghouls and the wailing haunts of six lost maidens seem to command otherwise. The characters are hired by the town elders to put a stop to this madness. The characters will soon learn that all is not what it seems as they unravel the secret of the Maidens of Moordoth.

Trouble at Karam's Claim
When the miners at Karam's Claim depleted the hills, they set their sights on deeper wares. They dug too deep, and uncovered an unusual orc clan lead by an exiled ogre lord. The characters are in the right place at the right time, and must rescue the miners from the clutches of evil.
A 1-sheet adventure designed for a party of four to six low-level Labyrinth Lord(TM) characters, though it can be easily adapted to any old-school fantasy role-playing game. This adventure is combat-heavy. If the players lack sufficient front-line fighters, consider having miners take up swords from fallen orcs and fall in line with them.

Planet Thirteen have produced the fun little dungeoncrawl, The Purple Worm Graveyard
It is said that when the largest and most ancient of purple worms know that the time of their death is near, they make their way through rock, earth, and water to the legendary Purple Worm Graveyard.
The graveyard is said to lie somewhere in the barren Rockspyre mountains, but its exact location is unknown. Now and then an adventuring expedition sets out on a hunch or clue seeking the graveyard. Most return empty-handed and dispirited. Some never return.
But a few, just enough to keep the legend alive really, come back with whispered tales of subterranean fields littered with a fortune in purple ivory, unguarded and ripe for the take.
The Purple Worm Graveyard is a short dungeon crawl adventure that's best for levels 1-4. The adventure has 15 entries and is designed to provide a variety of challenges for a new adventuring party or one-shot game. It's got fabulous treasures, cunning monsters, dangerous traps, and ancient mysteries for players to discover and explore. It's written using the Labyrinth Lord rules, but will work with any dungeon crawl adventure game.
